- Documentation of current enrollment
- A statement of interest.

A written description of an individual's educational background, research experience, and professional accomplishments.
Two (2) Letters of Recommendation
Applicants must identify references and request the LORs from within their Slate account. LORs must come directly from qualified individuals who are in a position to comment on an applicant’s knowledge, capabilities, and performance related to the appointment for which the individual is applying.
Proof of Enrollment
Transcript must include the following:- Institution Name
- Printed Date
- Student Information
- Name
- Date of Birth or Student ID
- Academic Degree/Major
- Dates of Enrollment (must cover the duration of the MD Anderson appointment)
- Spring Enrollment required for January – August appointments
- Fall Enrollment required for September – December appointments
The following documents will not be accepted as proof of enrollment:
- Computer printout
- Class schedule
- Payment receipt
For students
currently enrolled outside the United States, the following must be uploaded in Slate.

Certificate of Health Statement (CHS) - It is the policy of The University of Texas MD Anderson Cancer Center to require all observers or trainees entering the institution to receive vaccines for preventable diseases as specified by MD Anderson and based on the level of exposure and risk they present to patients. The CHS form must be completed and signed. Incomplete forms may delay your start date and some cases may result in rescinding of this appointment.
